2026 Community Health Resource Directories are available!

The St. Joseph Community Health Foundation is pleased to announce the release of the updated 2026 Community Health Resource Directory. As part of our mission to connect Allen County residents with quality care, the Foundation produces directories of free and low‑cost health and wellness providers that may be shared throughout the community. Printed copies of the 2026 edition are available at no cost for organizations, healthcare providers, community partners, and individuals.
The 2026 directory offers an expanded collection of local resources, including:
- Listings for more than 90 free or low-cost health and human service agencies serving Allen County, Indiana.
- Twelve organized service categories, including medical care, dental care, mental health, transportation, food assistance, pregnancy services, financial support, and more.
- Clear details for each agency, including location, phone number, website, hours of operation, bus routes, payment options, and eligibility requirements.
- Notation of language services available at each agency, including support provided through tele interpreters.
- A durable and convenient 5.5 by 8.5-inch printed booklet designed for offices, outreach teams, and resource tables.
The Foundation encourages partners to request copies early to ensure they have the most current information for the year ahead. Free copies and larger quantity orders may be requested at sjchf.org/directories.
