2026 Community Health Resource Directories are connecting Allen County to care

The St. Joseph Community Health Foundation is pleased to share the newly updated Community Health Resource Directory 2026–2027, a comprehensive guide designed to help Allen County residents find quality, affordable health and wellness services. Provided at no cost, the directory reflects the Foundation’s mission, rooted in the legacy of Saint Katharina Kasper and the Poor Handmaids of Jesus Christ, to ensure that every person has access to the care they need to live with dignity and hope.

The directory brings together an extensive collection of agencies offering free or low‑cost services across Allen County. Each listing includes essential details such as hours of operation, payment options, eligibility guidelines, bus routes, and available language services. This practical information helps individuals, families, and service providers quickly identify the most helpful resources and understand how to access them.

Organized for easy navigation, the directory features twelve major service categories, including:

  • Medical Services: Primary care, chronic disease management, specialty clinics, and more.
  • Dental Services: Preventive cleanings, X‑rays, fillings, dentures, and emergency care.
  • Mental Health & Addiction: Counseling, addiction recovery support, psychiatric services, and case management.
  • Immunization Services: Vaccines for children and adults, including free immunizations for eligible patients.
  • Health Screenings: Vision, hearing, and other preventive screenings for early detection and intervention.
  • Transportation Assistance: Agencies that help residents reach medical appointments.
  • Support & Education: Health education, chronic disease management classes, and community wellness programs.
  • Food & Nutrition: Food pantries, nutrition programs, and WIC services.
  • Financial Assistance: Programs that help with medical bills, prescriptions, and other essential needs.
  • Pregnancy Services: Prenatal care, childbirth support, and referrals, with additional multilingual resources available at PrenatalPath.com.

The directory is intentionally built to serve residents with diverse needs. Many agencies offer interpretation in Spanish, Burmese, and other languages, either on‑site or through tele‑interpreter services. Listings also note whether a government‑issued ID is required, whether walk‑ins are accepted, and what payment methods are available, including sliding‑fee scales, Medicaid, Medicare, and free services.

Nonprofits, clinics, schools, churches, and social service providers rely on this directory to guide clients toward the care they need. With more than 90 agencies represented, it remains one of the most widely used and trusted tools for connecting vulnerable individuals and families with essential health resources.
