The St. Joe Foundation provides the Community Health Resource Directory at no cost to the public and organizations supporting low-income and vulnerable residents.
Printed directory copies can be ordered by filling out the request form at SJCHF.org/directories. A PDF version is available on the same page for easy viewing or download.
The directory is updated and printed every other year and includes information on over 100 free or low-cost, high-quality healthcare services in Allen County, Indiana. Services are organized into twelve categories including medical, dental, mental health, immunization, transportation, and health screenings. Each listing includes details like services provided, hours of operation, accepted payment methods, if you need a photo ID, the nearest bus line, and if interpreters for other languages are available.
“The Foundation is dedicated to making sure everyone, especially those who are vulnerable, have access to quality, affordable health and wellness services,” said Mark Burkholder, projects & multimedia manager of the St. Joe Foundation. “Many non-profits tell us they depend on the directory to connect their clients with the affordable healthcare services they need.”
The Foundation gives out over 6,000 copies of the directory each year.
The St. Joe Foundation also prints and distributes the Prenatal and Infant Care Resource Directory (PIC). The 2025 PIC directory will be released in the fourth quarter of 2024, and limited copies of the current version are available until then.
